Believe it or not that mark on the earlobe is part of machine doubling or ejection damage. I've seen it on many machine doubled coins. Check to see if your coin has the machine doubling on either side. But I use that as part of my check for machine doubling. The mark under the ear, Not sure what that is. I had a 1993-D and it didn't have the mark under the ear. I just didn't have a Philly one handy to compare it with right now. Maybe someone else will.
