Die overpolishing - somewhat common. Polishing a die takes away the lowest relief as it appears on the coins first, since that's the highest relief on the die. All of the missing neck, missing throat, lips that float, filled ear, floating roof "varieties" are nothing more than this, and never really warrant attention because very few people would have interest in collecting them.
