The Fed Reserve infrequently would receive bags from a different district in shortage, and mix the bags in with everything else to go to the counting and rolling machine. While I have never encountered a separate mint new coin in a roll, I have opened rolls and received different dated coins in the middle of the roll. An OBW roll of 1987D yielded one brown 1979D cent in the middle of the roll.
Interesting find there Clackamas.
